For now the easiest way to make you questionnaire / task publicly accessible for inclusion in an ethics application or paper is with out open access repository Gorilla Open Materials (GOM). GOM then not only shows what you've done, but is also cloneable and reusable by other researchers which we believe is better than a PDF. We do appreciate that ethics boards may want something printed out for various reasons. We are looking into it!
